Vaccine Information

These Vaccines are available to individuals with an active ForwardHealth, BadgerCare or Medical Assistance card at check-in.
For uninsured individuals or families we just ask for a $2 donation per shot, but no one will be turned away for inability to pay.

Diphtheria, Tetanus, Pertussis (DTap, Td, TdaP - all combinations available)

Hepatitis A
Hepatitis B
Influenza for Children up to age 18
Measles, Mumps, Rubella (MMR)
Varicella (Chickenpox)

Adult Influenza Vaccine $25

Please note we are unable to bill a Medicare HMO or Medicare Replacement Plan.  We can bill regular Medicare Part B
